Why gather with communities?

There is a need to shift care models, from value-based ones at the hospitals to relationship-based ones in the community

Healthcare and social care systems often lack understanding of the unique contexts and needs of residents in rental communities, in order to support them effectively. These systems also often approach the community from a needs-based perspective, rather than a strengths-based one.

This gives rise to services, programs and initiatives that are mismatched with the community, which often also disempower them and further the power distance between residents and care professionals.
